Our nursing department staff works hand-in-hand with the patient, family members, and in collaboration with the care team to assess, prepare, and interpret a patient's individual care plan. At Darancare, we help our patients with physical or mental limitations achieve their primary goals - to maintain wellness and independence, and to preserve assets. With the right resources in place, along with individualized care in a familiar and warm environment, securing a patient's well being is much easier and more affordable. This new care model focuses on a proactive prevention program. Our home care system is designed to maximize our patient's abilities, which can help them delay or avoid institutionalization. A Comprehensive Geriatric Assessment helps diagnose and prioritize problems. We develop a short and long-term plan for prevention, treatment, and rehabilitation strategies that emphasize improving and maintaining patient function. Function can be influenced by biological, psychological, and social issues experienced by a patient. Our task is to identify those issues and find ways to eliminate problems that affect our patients in performing daily living activities. After identifying a patient's needs, our nursing staff implements the plan of care using all available resources.
